Why Your wRVU Report Doesn't Match Your Notes

William W. Bridge, M.D. — board-certified physician and medical director · 1 min read

Usually because charges never got logged, not because the notes fell short. A missed 99232 is 1.59 work RVUs that the documentation already supports. Keeping your own daily tally lets you reconcile before the month closes.

Where does the gap come from?

On my unit it is rarely the note. It is the charge. A patient seen Saturday, a note signed Monday, a charge that never dropped. Each missed 99232 is 1.59 work RVUs; each missed 99233 is 2.40 (CMS PFS relative value file, 2026).

Isn't the fix to document more?

No. The documentation was already there. The fix is capture: logging the code your note supports, the day you see the patient. That is accurate capture of documented work, nothing more.

How do I check the report?

Keep a tally of your own. Date, a generic label like "Patient 4," the code you billed. At month end, line it up against the report and send the missing ones to your coder with the dates. The complete wRVU guide covers what each common code carries.

I built Chartside's Tracking for exactly this tally: you log the code you chose and it adds up the day, with no names stored. What a single wRVU is worth is in what is one wRVU worth in 2026. The early-access list hears first on December 1.
